Street Vendor Registration

1. The first sheet contains the information you need regarding registration. Please make sure you print, sign, and return the waiver page with your registration.

2. The next two are the Certificate of Compliance and the booth registration form. Both must be printed and filled out to be registered as a vendor at Potato Days.
